Relinquishing a dog can be stressful for all concerned. By completing this Intake Checklist form in advance we can better prepare for your dog's arrival, while simplifying your trip to Homeward Bound.
Understanding That Surrender is Permanent: I/We warrant that we are the rightful owner(s) of the surrendered dog. I/We am/are signing (via digital signature on the Intake Checklist form) and relinquishing said dog of my/our own free will and not under duress of any kind, and acknowledge the surrendering of legal ownership and any and all future claims, including the right to reclaim the dog. Your submission of this form constitutes your electronic signature
